Participating with CHM means we all are more involved in the actual management of our healthcare--a really good thing. You are more involved in every decision with your care as well as how funds are allocated for you and your family's  benefit.

If you have recently submitted your application to participate with Christian Healthcare Ministries, email office@crossroadsonline.tv for your packet.

If you have an eligible medical expense to submit for reimbursement through Crossroad's HRA program, click here to print the reimbursement form needed to submit your receipts.

Because you now need to track your expenses to submit to CHM with anything over $500 per incident, here is a spreadsheet to use for that purpose (You must make a copy once you click the link so you are not sharing your health information with others).

If within a 90-day period, you spend $500+ on one incident, you will submit that to CHM. Remember that if you have already received reimbursement from Crossroad's HRA, you will need to return that once CHM has submitted payment to you. 

When you complete your application with CHM, you will receive an email with your member number and a portal access code. You will register first, and then be able to navigate to this portal each time you need to submit receipts and access any updates.

To look up information about your account with Christian Healthcare Ministries, go to the portal and login with your specific credentials.
